How do I know which form to fill out?

You are required by law to file a report within 24 hours with the City or County Board of Health. Follow the links below based on where the bite occurred. If the bite occurred within the City of Cincinnati, click to file with them. If it were outside of the City of Cincinnati city limits, file with Hamilton County. You DO NOT need to file a report with Cincinnati Animal CARE.
